The Director of Patient Management is accountable for the implementation, coordination and management of the Department.  
Direction of the Department will include clinical, financial, quality and HR management as well as program planning, education, monitoring, and maintenance.  
The Director of Patient Management is responsible for this program that transitions patients through the continuum of care in a timely and cost effective manner. 
The Director also has oversight of the Patient Advocate program. 
The Director of Patient Management provides services to patient throughout the lifespan including newborns, children, adolescents, adults and geriatric age groups.
The Director will be available to provide services in any area of the hospital including (but not limited to) the Post-Anesthesia Care Unit (PACU), the Emergency Department (ED), Criticial Care Units (ICU and CCU), Telemetry Units, Medical/Surgical Units, Women’s and Children’s Units (L&D, PEDS, NSY, NICU), the Alzheimer’s Unit (ACE), and Out-Patient Services (observation units, Cath Lab, same day surgery, etc.).
Must have a BSN and Master's degree is preferred.
Must have a RN license in New Mexico or compact RN license is required.
Looking for someone with 5-10 years experience in nursing and/or prefer certification in a related field to Case Management.
Facility is over 200 beds.
The person will be over around 15 employees and the mix is: Director, Adm Asst, 6RNs(2 are team leads),3 Social Workers (1 LMSW, 2 LCSW) plus adding a couple
It is an integrated model and plan to implement an ED model with 2 FTEs covering 10-12 hours/7 days/week
The primary focus for the Director is: Length of stay, Readmissions and Denials

The facility is offering a competitive hourly range between $33.81-$50.72 depending on experience. They also offer a great benefit package.
